Levels of Biohazard
Blood and other biohazard materials can contain bacteria, viruses, and other microbes of varying danger levels, and it’s important for professionals to know how to best protect themselves when confronted with any of these materials. The United States Centers for Disease Control and Prevention (CDC) categorizes the potential diseases caused by these microbes into levels of biohazard risk, from level 1 (minimum risk) to level 4 (extreme risk). Laboratories and other facilities are categorized, as well, so that each lab is appropriately equipped with safety measures according to the level of biohazard it deals with. Lab safety levels are known as BSL (Biosafety Level) 1-4 or as P1 through P4 (Pathogen or Protection Level).
• Biohazard Level 1 involves bacteria and viruses that do not usually cause disease in healthy human adults. These substances include Bacillus subtilis, canine hepatitis, Escherichia coli, and varicella (chicken pox), as well as some cell cultures and non-infectious bacteria. At this level, only minimal precautions like gloves and facial protections (masks and goggles) are needed.
• Biohazard Level 2 refers to bacteria and viruses that cause only mild disease to humans or that are unlikely to become aerosol threats in a laboratory setting. Some examples are hepatitis A, B, and C, certain strains of influenza A, Lyme disease, salmonella, mumps, measles, scrapie, dengue fever, and HIV. Professionals can safely perform routine diagnostic work with these clinical specimens as long as they follow appropriate safety practices and procedures. Research work using level 2 bacteria and viruses (including co-cultivation, virus replication studies, or manipulations involving concentrated virus) can be done in a BSL-2 (P2) facility while using BSL-3 practices and procedures.
• Biohazard Level 3 includes bacteria and viruses that we have vaccines and treatments for but that can otherwise cause severe to fatal disease in humans, such as anthrax, West Nile virus, Venezuelan equine encephalitis, SARS virus, MERS coronavirus, hantaviruses, tuberculosis, typhus, Rift Valley fever, Rocky Mountain spotted fever, yellow fever, and malaria. Some parasites like Plasmodium falciparum and Trypanosoma cruzi, which cause malaria and trypanosomiasis respectively, also fall under this level.
• Biohazard Level 4 is where professionals handle viruses and bacteria that cause severe and even fatal disease in humans without the protection of vaccines and available, effective treatments. Examples of these diseases include Bolivian and Argentine hemorrhagic fevers, Marburg virus, Ebola virus, Lassa fever virus, Crimean–Congo hemorrhagic fever, and other hemorrhagic diseases and rishibola. One exception is the Variola virus (smallpox), which falls under level BSL-4, even though we have a vaccine against it.
When dealing with biological hazards at this high-risk level, laboratory workers are required to wear a positive pressure personnel suit with a segregated air supply for their utmost protection. In addition, the entrance and exit of a Level 4 biolab contain multiple showers, a vacuum room, an ultraviolet light room, an autonomous detection system, and other safety precautions designed to destroy all traces of the biohazard. Multiple airlocks are employed and are electronically secured to prevent both doors opening at the same time as another safety measure. All air and water service going to and coming from a Biosafety Level 4 (P4) lab will undergo similar decontamination procedures to eliminate the possibility of an accidental release.
